The privacy of your personal information and information about your pets is very important to us. This Notice describes how our practice may use and disclose personally identifiable information regarding you and your pets (“Information”). This notice is applicable to both your on-line and off-line privacy.

- To provide, coordinate, and manage the care of your pets. For example, we may use and disclose Information if you need lab work or x-ray services that we do not provide.
- To bill and collect payment for treatment and services provided to you. We may share portions of your Information, for example, to collection agencies, attorneys, consumer reporting agencies, and veterinarian care insurance companies and plans.
- To manage our operations and business activities. For example, we may use your Information to identify and make improvements to our business operations, or to identify groups of pets with similar health problems. We may contact you with information about treatment, services or product options. We may also use outside consultants to help us improve our practice, and certain Information may be accessed by such consultants.
- As required by law or for public health activities. For example, we may be required to report certain diseases to public health departments.
- In a format where all personally identifiable information about you or your pet has been removed.
- To provide you appointment, vaccination, and other reminders.
- To the successor of our practice, if we were ever to sell our practice to another owner or veterinarian practice.
We may contract with third parties to provide services to our practice. When Information is disclosed to third parties for this purpose, then our contract with the third party will contain provisions and requirements that obligate the third party to maintain the privacy and security of the Information.
